I understand that participation in the SMART Super Session at the Hyannis Yacht Club has inherent risks for my child that may include severe injury, fractures and drowning. Every effort has been made to make the program safe; nevertheless, accidents can happen.

I understand that all sailing students must come to class with a Coast Guard approved floatation device with a whistle attached. They must be in good condition and the correct size for the student. All students must wear shoes that cover the toes and encircle the entire foot.

I give my permission for the above named student to participate in all of the activities of the Hyannis Yacht Club SMART Super Session.

I attest to the fact that the child is in good health. Furthermore, I grant permission to the instructors of the Hyannis Yacht Club to administer any first aid that may be deemed necessary in the event of illness or injury. I likewise release, absolve, indemnify and hold harmless the Hyannis Yacht Club, its members, agents and employees from any and all liability, loss, damage and expense, which may result from any participation in the above programs.
